Registration Guidelines and Forms

Deadline: post marked by Friday, Dec. 18, 2009.
Please get all paperwork to us ASAP.
Fax over all forms prior to mailing in order
to secure your spot in the tournament.

Registration Guidelines:

• 1. FORMS: College Intramural Director must authorize eligibility of roster participants.
  The Registrar must verify student status and enrollment (stamp official seal) on the original Team Roster Form. All deadline dates will be enforced. All players, managers, and coaches must appear on the roster form and pay appropriate fees. Return completed forms (Commitment Application, Payment Form, Sealed Team Roster, Signed Release Form) and appropriate fees to:

• 2. ACIS Regional Winners: please make all your arrangements for travel stipends and entry fees through ACIS contact Suzie Whelan at (440) 602-9671 x100. Regional Winners must arrange their own hotel accommodations (see hotels); as well as completing all necessary registration forms for the tournament (see 1. FORMS above). Regional winners are ONLY allowed to substitute up to 4 players from their original Regional Tournament roster.
  
  
• 3. College Roster Change Policy: No refunds are given for team members dropped from a roster. Players must be added to a roster by start of the first game with appropriate fee, plus $10 per addition using the Team Roster Additions Form. The form must be verified by the Intramural Director and bear the Registrar's original seal. Form may be faxed ONLY if the original form on file bears the Registrar's seal. All rosters are frozen at the start of your first game.
  
  
• 4. Open Roster Change Policy: No refunds are given for team members dropped from a roster. Players must pay $10 per change/addition using the Team Roster Additions Form.

In order to improve legibility; all forms are designed in a PDF format
which allows you to type necessary information directly on the form.
After opening form, we strongly encourage you to type the information
in the highlighted boxes; then you may print the form to complete the
other areas.
